Abbeyfield Southern Barossa Home Care Packages is now officially the provider of Consumer Directed Care Services as part of your Home Care Package.  These packages enable eligible people to have a choice of how, when and what services they require to maintain their goals and lifestyle.  Abbeyfield can also help you to gain access to other services of your choice that will enable you to remain living independently.

HOW TO OBTAIN A HOME CARE PACKAGE

My Aged Care

To obtain a Home Care Package clients must first check for eligibility by getting an Aged Care Assessment - You can access more information by calling My Aged Care on 
1800 200 422 or through the My Aged Care website by clicking here - ACAT 

Once you have been assessed as eligible for a package you will be informed in writing, you will then need to register on the My Aged Care website.  Please call Abbeyfield if you require assistance with this.

If you have an existing package you can choose your provider and transfer from an existing provider if you wish. Nominate  Abbeyfield Southern Barossa Home Care as your preferred provider.

WHAT SERVICES CAN SOUTHERN BAROSSA HOME CARE PROVIDE

  • CLINICAL: Wound dressing, medications
  • PERSONAL CARE: Showering, dressing, mobility, meal preparation and  eating

  • SUPPORT SERVICES: Laundry, house cleaning, gardening, home maintenance, transport to help you shop, visit your doctor or attending social activities
  • SUPPORT STAFF: Administrative and Clinical support staff who can assist you with your enquiries

HOME CARE PACKAGE FEES & FUNDING

The Australian Government, Department of Social Services funds Consumer Directed Care/Home Care Packages.  The funding available to you will be defined by the package level that you are assessed for.  All Home Care Packages operate on an individualised and transparent budget, provided to the consumer on a monthly basis.  The budget outlines details of the Administration and Case Management costs and Service and Support provision and/or purchasing costs.  Generally, people are required to contribute to their package by paying a basic daily care fee.  Exact fees are based on your individual circumstances and income and will be clearly documented and explained as part of your care package.
